Please collect and prepare and send the following documents:

1. You must send your valid US Passport with at least 6 months of validity left and one blank visa page.

2. Three Passport Photos. They must be 2x2, color or black and white with a plain light or white background and show a full front view of face.

3. Two copies of the Visa Application Form (signed and completed)

4. A business letter, detailing purpose of trip, name and address of contact in Bangladesh, and that your company will be financially responsible for you while in Bangladesh and guarentee transportation back to the U.S. This must be signed by an officer of the company other than the traveler.

5. Important: You must fill out the American Passport Express Customer Information Form

6. Copy of either airline tickets or flight itinerary

7. Processing Fees:

One Money Order made payable to the Consulate General of Bangladesh (see table below) Do not send personal or company checks. Include your name and date of birth on the money order. Do not include your address on the money order.

One check or money order made payable to American Passport Express for:

Rush Visa Processing $70 -leaving in 15 days or less

Standard Visa Processing $55- leaving in more than 15 days
