1. You must send your valid US Passport with at least 6 months of validity left and one blank visa page.

2. One Passport Photo. Photo must be 2x2, color or black and white with a plain light or white background and show a full front view of face.

3. Visa Application Form (signed and completed)

4. A business letter, detailing purpose of trip, name and address of contact in Belarus, and that your company will be financially responsible for you while in Belarus.

5. Official invitation from company in Belarus.

6. Please Fill Out the American Passport Express Customer Information Form

7. Processing Fees:

One Money Order made payable to the Embassy of Belarus (see table below) Do not send personal or company checks. Include your name and date of birth on the money order.

One check or money order made payable to American Passport Express for:
