1. You must send your valid US Passport with at least 6 months of validity left and one blank visa page.

2. One Passport Photo. Photo must be 2x2, color or black and white with a plain light or white background and show a full front view of face.

3. Visa Application Form (signed and completed)

4. Please Fill Out the American Passport Express Customer Information Form

5. Medical insurance is required for all foreign citizens travling to the Republic of Belarus. This medical insurance can be obtained upon arrival at the border of entry.

6. Confirmation of hotel arrangement from an authorized Belarus travel company showing reference number of your visa.*

* If you are not staying at a hotel, you will need a letter of invitation to the Belarus. Call American Passport Express if you need assistance with this invitation.
