Please collect and prepare and send the following documents:

1. You must send your valid US Passport with at least 6 months of validity left and one blank visa page.

2. Two Passport Photos. They must be 2x2, color or black and white with a plain light or white background and show a full front view of face.

3. Two copies of the Visa Application Form (signed and completed)

4. A business letter, detailing purpose of trip, name and address of contact in Central African Republic, and that your company will be financially responsible for you while in Central African Republic. This must be addressed to the Embassy of Central African Republic, Visa section and signed by an offical of the company other than yourself.

5. Important: You must fill out the American Passport Express Customer Information Form

6. International certificate of Vaccination for Yellow Fever.

7. Processing Fees:

One Money Order made payable to the Consulate General of Central African Republic (see table below) Do not send personal or company checks. Include your name and date of birth on the money order. Do not include your address on the money order.

One check or money order made payable to American Passport Express for:
