Question #1.3: This must be completed in Chinese characters if you are a former Chinese citizen.
Question #1.5: You must complete this section if you have ever been married or had a legal name change. If your name has never changed, then please write "none" in this field.
Question #1.10 thru 1.13: This question must be answered, unless American Passport Express is obtaining the passport for you. If American Passport Express is obtaining the passport for you, please leave this field blank.
Question #1.14: This question must be answered. Please list your occupation, or check "other" and specify an occupation. Please also include the name, and phone number of the company in the U.S. that employs you.
Question #2.1: You must only check off the word "business" in this field.
Question #2.5: This question must be answered in full. Please list multiple destinations in China if applicable. Do not list Tibet as a destination unless you have the necessary special authorization to travel there. This authorization would be supplied by the organization in China sponsoring your trip, and must be supplied with the rest of your application materials.
Question #4.7 thru 4.9: You must provide the name of the Chinese company that is inviting you. You should also include contact information for the company.
