You must send your valid U.S. passport. The passport must have at least 6 months of validity left, and have at least 1 blank visa page. (Blank endorsement pages in your passport are not acceptable.)

Photo must be 2x2, black and white, or color, with a white, or light background, and show a full, front view of your face. The photo must also be printed on photo quality paper, photos printed on normal paper will be rejected by the embassy.

2. Former Chinese citizens must fill in their Chinese name in Chinese characters on the application form.

3. Please check off that the purpose of your trip is "tourism" in question #2.1 of the application.

4. If your occupation as listed in question #1.14 is at all related to the media (journalist, columnist, correspondent etc.) please include an additional letter stating that you are travelling to China for tourist reasons only, and will not be engaging in any activities relating to your occupation.

5. In sections 4.7 thru 4.10, please list your relationship to the inviting person. If you are being invited to China by an organization, not an individual person, please leave these selections blank.

1. If you are applying for a single, or double entry visa, please continue to part E. below.

2. If you are applying for a multiple entry visa, you must meet one of the following requirements:

• You are able to submit documentation certifying that you will be visiting china frequently for tourist reasons. This can include a letter of invitation from a family member in China, or proof of kinship, an ownership certificate for real estate in China, sales or lease agreement for an apartment in China, a copy of an adoption certificate, or child's former passport if he/she was adopted in China.
• You were born in China, but no longer maintain citizenship, and have visited China at least twice within the last year. You must be able to submit copies of your previous Chinese visas.
• You are able to submit an official invitation letter from the host institution in China, or an application letter explaining the need to visit China frequently for tourism.
• You are renewing a 6 month, or 1 year visa, and can submit a copy of the previous visa if it is in a different passport that the one submitted for the new visa.

You will need to submit a copy of round trip airline tickets or itinerary showing round trip flight numbers and times, or a cruise itinerary showing round trip boarding information. Please make sure the following information is also included on any proof of departure documentation:

• Confirmed travel to China within 90 days of submitting the visa application
• The applicants name
• The date of entry into China
• The date of departure

A hotel reservation is also required for obtaining a Chinese visa.

1. Consular Fees: This fee must be paid by money order or company check made payable to the Chinese Embassy. DO NOT SEND PERSONAL CHECKS. Include your name and date of birth on the check or money order. If paying by money order, do not include your address on the money order. This fee can also be paid by credit card, using the customer information form in step #2 below.
