You must send your valid U.S. passport. The passport must have at least 6 months of validity left, and have at least 1 blank visa page. (Blank endorsement pages in your passport are not acceptable.)

Photo must be 2x2, black and white, or color, with a white, or light background, and show a full, front view of your face. The photos must also be printed on photo quality paper, photos printed on normal paper will be rejected by the embassy.

This form will be secured for you by the company that is employing you in China.

Please submit one of the following documents to satisfy this requirement:

1. A Work Permit for Aliens issued by the Chinese Labor Ministry.

2. A Foreign Expert's License issued by the Chinese Foreign Expert's Bureau.

If you are applying for a work visa, and family members will be accompanying you to China, they must provide one of the following:

1. A visa notification issued by the authorized Chinese unit.

2. Proof of kinship in the form of an official marriage, or birth certificate.

1. Consular Fees: This fee must be paid by money order or company check made payable to the Chinese Embassy. DO NOT SEND PERSONAL CHECKS. Include your name and date of birth on the check or money order. If paying by money order, do not include your address on the money order. This fee can also be paid by credit card, using the cumtomer information form in step #2 below.
