Please collect and prepare and send the following documents:

1. You must send your valid US Passport with at least 6 months of validity left and one blank visa page.

2. Two Passport Photos. They must be 2x2, color or black and white with a plain light or white background and show a full front view of face.

3. Two copies of the Visa Application Form (signed and completed)

4. Two Copies of a business letter, detailing purpose of trip, name and address of contact in Democratic Republic of Congo, and that your company will be financially responsible for you while in Democratic Republic of Congo. This must be addressed to the Embassy of Democratic Republic of Congo, Visa section. These must each have an original signature.

5. Important: You must fill out the American Passport Express Customer Information Form

6. Notorized invitation from the Democratic Republic of Congo.

7. Processing Fees:

Mode of Payment: Money order, certified check or company check payable to the order of Embassy of the Democratic Republic of the Congo. Include your name and date of birth on the money order. Do not include your address on the money order.

One check or money order made payable to American Passport Express for:
