Please collect and prepare and send the following documents:

1. You must send your valid US Passport with at least 6 months of validity left and one blank visa page.

2. Two Passport Photos. They must be 2x2, color or black and white, with a plain light or white background and show a full front view of your face.

3. Two copies of the Visa Application Form (signed and completed)

4. Important: You must fill out the American Passport Express Customer Information Form

5. Copy of Itinerary or plane ticket from either an airline or travel agent showing exit from Democratic Republic of Congo.

6. Notorized letter from the Democratic Republic of Congo.

7. International certificate of vaccination for Yellow Fever.

8. Processing Fees:

Mode of Payment: Money order, certified check or company check payable to the order of Embassy of the Democratic Republic of the Congo. Include your name and date of birth on the money order. Do not include your address on the money order.
