Express Service Instructions - Adding Visa Pages to a Current US Passport

The Derpartment of State requires that you will fill out the government application online for expedited
Passport cards are currently not being issued. Selecting that you would like a passport card on your application maybe cause for your application to be rejected.

Fill out and create DS-4085 form here

No alterations can be made to the original signature and date on the government application. Example: White out, scratch marks, or scribbeling ect. Any changes made to the original signature or date will result in the need for an additional completed application.
State Department regulations state that the application (DS-4085) has to be submitted within 5 days of when it is dated. Please date the application for the day American Passport Express will receive it.

Please contact American Passport Express at 1-800-455-5166 and ask the representative to set up an Express Reservation

Once your reservation has been set up, you will be directed to print two copies of one of the letter of authorization (L1-L11) listed below. The letter of Authorization gives us permission to process your passport application

The two copies of the letter of authorization must:

• Have original signatures by the passport applicant
• Both be signed using a blue ink pen if possible
• Not be photocopies or faxes

• Two copies of your plane ticket showing departure date or
• Two copies of your itinerary from the travel agent or airline

Personal checks, money orders or use the customer information form (see step 2 below) to pay all fees by credit card.

1. Government expedite fee made payable to "US Dept. of State" for $60 (write your Date of Birth on this check.) Click Here for more information about this fee.

2. American Passport Express Service Fee.

Made payable to "American Passport Express" for $99.

3. Return Shipping Fee

Return shipping of your passport (choose one):

• Include a prepaid FedEx airbill or
• Add $25 to the American Passport Express fee for overnight shipping

All applicants must complete and submit the customer information form.   After submitting the form online print a copy of the "confirmation page" which appears. The form gives our company information which is used to help accurately process your passport order.

When you have items from steps 1 & 2 above, call American Passport Express (1.800.455.5166) for the Address to Send your package.

-Please do not seal the shipping envelope until you get the address from an American Passport Express representative.

On Your FedEx airbill Mark the box that says "Priority Overnight" (Next Business Morning)

You will receive email confirmation with your order number and status the day we receive your application.
